Fat grafting of the labia majora: A 47-year-old female presents with thinning of the labia majora for fat grafting. Physical examination shows fat atrophy of the mons pubis and labia majora in both the standing and lithotomy positions. Before and after pictures taken at 4 months after 40 cc fat grafting to the mons pubis and labia majora.
The proposed mechanism for vaginal and urethral changes seems to be excess
collagen production is response to thermal injury. Histological studies have shown an increase in organized collagen fibrils as well as an increase in blood vessel numbers in vaginal specimens after treatment with these devices. At this time, however, most clinical studies have been based on subjective pre- and postoperative questionnaires given to patients before and after the procedures. Clinical outcome studies are underway to further assess the efficacy of laser and RF vaginal therapy.
CONCLUSIONS
Female aesthetic genital surgery has increased significantly over the past 10 years. Women of all ages show interest in improving the appearance of their genitalia. The popularity of Brazilian waxing and the availability of the Internet as an anonymous source for research have contributed to the exponential growth of the field.
Many publications in both the plastic surgery and gynecology literature exist with informative descriptions of surgical techniques to address labia minora excess. The two most common labiaplasty techniques are wedge reduction and edge trim. Physical examination and surgeon experience should determine the most appropriate approach to labia minora reduction. Outcome studies have shown a low complication rate and high patient satisfaction postoperatively.
Other areas of the vulva may be considered for cosmetic rejuvenation as well. Patients may complain of excess skin or fat of the labia majora. Surgical reduction of the labia majora or noninvasive laser or RF procedures may result in significant cosmetic improvement of the vulvar region.
