Cryotherapy has also been described for the treatment of lentigo maligna. While clearly excision is preferable, the size, location, and comorbidities sometimes make excisional surgery impractical. At least three small case series describe cryosurgical treatment of lentigo maligna. One series of 30 patients with lesions measuring 1.3 to 4.5 cm treated between 1977 and 1992 reported two recurrences (6.6%).29 This study utilized a double freezeโthaw cycle with 1-cm lateral spread. A study of 20 patients reported 2 recurrences (10%)30, while another study of 20 patients reported 3 recurrences (15%).31 Contemporary dermatologists may treat lentigo maligna with cryotherapy less frequently due to both advances in reconstructive techniques as well as concerns with sampling error. Without the opportunity for histological evaluation, the presence of microscopic foci of lentigo maligna melanoma (which may occur in a significant proportion of cases) cannot be identified, and prognostic outcome cannot be predicted accordingly.
