若受影響的體表面積 (body surface area, BSA) 不大於約 0.5%，則去頂術 (deroofing) 極為適合作為使用局部麻醉 (local anesthesia) 的「門診處置 (office procedure)」。29 局部麻醉通常分兩個步驟進行。25 首先，注射含有 lidocaine 與 epinephrine 的麻醉劑以浸潤周圍區域。如先前所述，添加 sodium bicarbonate 可減少注射時的灼燒感。21 此外，可將同一種局部麻醉劑注射入竇道 (sinus tract)。將 methylene blue 注入竇道可用於評估病灶的範圍。當病人偏好如此，或在發炎範圍廣泛或病灶多發的情況下，此術式亦可在程序性鎮靜與止痛 (procedural sedation and analgesia, PSA) 或全身麻醉 (general anesthesia) 下進行。
