玻尿酸酶 (Hyaluronidase)
hyaluronidase 是一種可分解玻尿酸 (hyaluronic acid) 的酵素,5 玻尿酸為存在於真皮 (dermis) 中的一種酸性黏多醣 (acid mucopolysaccharide)。當加入局部麻醉中時,hyaluronidase 可促進麻醉劑通過軟組織 (soft tissues) 的擴散。其結果為局部麻醉範圍更廣、起效更快、組織變形 (tissue distortion) 更少,以及組織游離 (tissue undermining) 更為容易。26 在眼科 (ophthalmology),hyaluronidase 添加物可使外科醫師減少注射部位、降低麻醉劑用量,並減輕病人瘀青 (bruising) 的風險。5 皮膚手術 (cutaneous surgery) 中的劑量各有不同。某些作者建議將 150 units of hyaluronidase 混入 20 to 30 mL of lidocaine,以達到 7.5 units/mL 的濃度。27
使用 hyaluronidase 也有一些風險,包括麻醉持續時間縮短,以及因吸收增強而導致毒性 (toxicity) 風險增加。27 hyaluronidase 也含有防腐劑 thimerosal,5 這是一種接觸性過敏原 (contact allergen)。對於高度敏感的病人,建議在使用前先行術前皮膚測試 (preoperative skin testing)。5,27 hyaluronidase 也不應使用於有蜂毒過敏 (bee venom allergy) 紀錄的病人。28
