Jowls are a telltale sign of aging that can be exacerbated by the appearance of volume loss in the pre- and postjowl sulci. Restoring volume loss can aid in creating a more defined and linear jawline. Indirect jowl improvement may be attained with fillers in the postjowl/mandibular angle and preauricular areas. Patients can benefit from the โ€œNefertiti liftโ€ using BTX-A to contour the jawline.133 Masseter hypertrophy can be ameliorated by BTX-A to attenuate lower facial rectangular appearances and attain a more triangular feminine facial appearance. A Cochrane review in 2013 could not identify any clinical trials assessing intramasseteric BTX-A134; however, clinical trials were subsequently published.135 A 24-week double-blind randomized split-face trial of 20 women showed the efficacy of both rimabotulinumtoxin B and onabotulinumtoxin A in treating masseter hypertrophy. Rimabotulinumtoxin B at a dose ratio of 70:1 to onabotulinumtoxin A had a shorter duration of action than onabotulinumtoxin A.135 It is prudent not to reduce masseter bulk if there are signs of jowls, as doing so may lead to jowl accentuation.
