This procedure is a useful approach for surgery of fingers two, three, and four.2,9 Due to anatomic variation, this approach is not advised for surgery on any other digit.

Its advantages are its fast onset (2 to 5 minutes) and the absence of risk to neurovascular bundles, as the needle is inserted on the palmar aspect of the digit (Fig.

34-6).2 A rare complication is injury to the flexor tendon that can result in scar or trigger finger.
