Lethal complications of Mohs surgery are rare, but two cases of cerebral emboli have been described following MMS for scalp tumors.52 To prevent this complication, patients with exposed calvarium (stripped periosteum) require careful patient positioning in the supine or prone position to limit negative pressure gradients that may occur via the noncollapsing veins of the head and neck such as the epiploic veins and the emissary veins that drain to the dural venous sinuses. The majority of neck and scalp incidences of air embolism occur via the posterior venous complexes; therefore, caution should be taken with Mohs defects with stripped periosteum on the posterior scalp to keep these patients flat during surgery and while waiting in between stages.53
