Postoperative dermabrasion is helpful in approximately one-third of cases. This can remedy any residual pits that may develop on sebaceous or rhinophymatous noses. Dermabrasion can be performed 6 weeks to 3 months postoperatively, or even later if desired.

First, hypertrophic areas are marked with a pen prior to anesthesia. Second, hypertrophic sebaceous hyperplasia surrounding the surgical lines is tangentially shaved, holding the scalpel almost parallel to the skin surface. Finally, sterilized 80-grit sandpaper is used to smooth the entire cosmetic unit. Patients apply petrolatum ointment and a nonstick bandage daily for 2 weeks, and are counseled not to allow the wound to develop a scab.

CONCLUSIONS

The bilobed flap provides a unique approach to nasal closures, as it combines the benefits of a rotation flap, transposition flap, and Z-plasty. With meticulous attention to design and execution, this technique can represent a fundamental approach to closures on the lower nose and, occasionally, elsewhere on the head and neck.
