The main reason for any inferior displacement of the ipsilateral alar rim is the lengthening effect of the Z-plasty. Larger angles in a Z-plasty result in greater gains in length.9 Thus, bulldozing is most likely to be encountered when employing larger flaps that require commensurately larger angles, and pivot within less mobile skin.12 Since this inferior displacement is noticeable intraoperatively, the first sutures on the primary lobe can be adjusted, or the primary lobe of the flap can be trimmed. Minor displacement of the alar rim usually corrects itself by 6 weeks.13
