The set-back dermal suture

The set-back dermal suture

This approach is frequently used in areas under significant tension, and as an alternative to the buried vertical mattress as a standard buried technique.3 Since it is easier to place than a buried vertical mattress suture, this technique may be used by budding surgeons, medical students, and residents as the workhorse technique for deep tension-relieving sutures (Figs. 13-4 through 13-6).

Figure 13-4. The needle is inserted through the underside of the dermis, exiting again through the underside of the dermis set back from the wound edge.

Figure 13-5. This is repeated on the contralateral wound edge.

Figure 13-6. Cross-sectional view demonstrating the path of the suture material through the dermis and the effect on wound eversion.
