Once the markings are completed, 1% lidocaine with epinephrine 1:100,000 with sodium bicarbonate is injected using a 30-gauge needle submucosally along the markings. Slow injection is important to reduce discomfort. After the mucosal incisions have been anesthetized, the lateral labial incisions may be injected. It is important to allow at least 10 minutes for the anesthetic and epinephrine to take effect. During this time, a sterile prep is performed on the perineum. The markings should be retraced as they tend to fade once the prep is complete.
