Marionette lines and the concavity along the labiomental line commonly develop as a feature of aging. The etiology stems from a number of factors including volume loss in the supporting structures such as the cheeks and mandible. Correction and effacement is desirable so long as it doesnโt masculinize the female face or create unusual fullness in the region. Indirect correction can be achieved with tissue augmentation at the angle of the mandible, preauricular area, and cheeks. Direct melomental rhytid correction can be accomplished with subdermal and subcutaneous filler placements.126 A downturned mouth is another aging feature that can be addressed with BTX-A in the depressor anguli oris (DAO) and fillers to lift the corners of the mouth.127
