Juvederm Voluma correction lasted 24 months for the anteromedial cheeks.77 Product placement should be aimed toward the premaxillary fat just above the body of the maxillary bone, deep middle fat pad, and inferior orbicularis fat pads. Small bolus placements using sharp 30 gauge or smaller needle injections are more precise but associated with higher risk of bruising and trauma to the infraorbital neurovascular bundle. Cannulas have been used for midface product placement. Two of the three

insertion points described by Surek et al. may be used to approach the anterior cheek. Port placement within a point created by the intersection of a horizontal line drawn from the nasal ala and intersecting with the nasojugal groove provide access to the middle superficial fat compartment. The other insertion point is located 1.5 cm inferolateral to the alar base in the nasolabial fold allowing access to the medial cheek fat compartment.84 The injector can use blunt cannulas for small boluses or fanning threads within the premaxillary fat, deep middle fat pad, and inferior orbicularis fat pads. Alternatively, a portal of entry approximately 3 cm lateral and 1 cm superior to the oral commissure can be used. Cannulas can be 22 or 25 gauge and 1ยฝ or 2 in long.29 For more precise cannula product placements in the deep middle fat pad, it is often helpful to pinch the medial cheek to lift the superficial middle fat pad, allowing better access to the deep middle fat pad.

Precautions, pearls, and pitfalls The infraorbital neurovascular bundle is a danger zone located on a line connecting the lateral palpebral commissure to the nasal ala. The closest mean distance between the infraorbital foramen and the infraorbital margin was measured as 8.8 ยฑ 1.0 mm.86
