The skin is very thin in this area and caution must be used to avoid clumping of product. Thus, HA products with hydrating properties and minimal lift capacity are desirable. Juvederm Volbella, Restylane Refyne, Restylane Fynesse, or Belotero Balance are a few examples of soft-tissue fillers that can be used in this area.

While blunt cannulas may be preferred in this area, some injectors use a serial microdroplet needle technique. When using a cannula, an entry point in the lateral aspect of the upper eyelid, 1 to 2 mm below the lateral edge of the brow, may be used. Filler placement should always be in the suborbicularis oculi plane. A 27- or 25-gauge, 1ยฝ-in cannula running superior to the primary fold and in the suborbicularis oculi plane of the superior sulcus with microaliquot placement of the desired filler should be used.40,41 The โ€œwalk the rim, feel the boneโ€ needle injection technique is advocated by some injectors for superior sulcus filling. The injector palpates the orbital rim to avoid the supraorbital and supratrochlear notches. The needle is inserted until the tip touches the bony rim. Once bone is felt, the needle is withdrawn slightly to so that a small amount of filler can be injected into the preperiosteal space just superficial to the bone. Multiple injections are given along the superior orbital rim, starting medially and moving laterally if necessary.
