Step six: Skin excision
The skin excision phase removes excess skin that has overlapped the incision after key suture retraction and stabilization, and can be performed with scalpel or scissors. When trimming hair-bearing incisions, the scalpel is once again beveled to the same angle as the initial incision to allow a thin interface at the incision junction which permits easier regrowth of the hair follicles through the incision. In the perfect facelift incision, hair remains on both sides of the incision line.
The remainder of the anterior and posterior auricular skin excess is removed as accurately as possible leading to a passive interface (Fig. 74-14). Wound tension will create unfavorable scars, and all incision interfaces must be closed without tension.
