Patient expectations

A facelift will improve excess neck skin, jowling, and platysmal banding. It will not address the central oval of the face, perioral wrinkles, periorbital wrinkles, or forehead wrinkles. Patients with large skin and/or fat excess or those with weak chins or oblique submental anatomy may have less impressive results. A true facelift will generally last a decade if performed in a comprehensive manner. In some patients, the result may last longer, while in others the effects may wane more rapidly, depending on lifestyle, heredity, and maintenance.

It is important that patients appreciate that a facelift is not a procedure designed to treat wrinkles. Although some facial and neck wrinkling will be improved, concomitant or post-facelift skin resurfacing is generally required. Facelift surgery can be very comprehensive, and complications may include nerve and vessel damage, hematoma, flap necrosis, lack of anticipated result, and relapse. A comprehensive, informed consent process (see Chapter 3) is an absolute prerequisite. It is imperative that any cosmetic surgery patient understands what their procedure will and will not accomplish.
