Superior sulcus deformity or hollowing of the upper sulcus can occur following blepharoplasty if too much fat is removed. With age, the central fat pad atrophies, while the medial fat pad becomes more prominent. Recent awareness about the importance of fat preservation in the upper lid has led to a reduction in fat excision, especially centrally and laterally. Overaggressive resection may lead to hollowing of the superior sulcus, which produces an aged appearance. The location and extent of fat excess should be carefully documented preoperatively, and minimal fat excision and/or repositioning should be performed intraoperatively.151
