Be wary of the patient that wants only eyelid surgery, but also needs a brow lift. These patients may have the appearance of excess eyelid skin, but they also have brow descent and usually have less dermatochalasis when the brow is manually lifted during the preoperative evaluation. Brow ptosis after blepharoplasty occurs from unrecognized preoperative brow ptosis, but can also develop from excess skin removal. If skin is removed in the setting of brow ptosis, the brow is often pulled further downwards postoperatively. Again, 20 mm of anterior lamella should always be left remaining after blepharoplasty. If both a brow lift and blepharoplasty are planned, the order of procedures is important. An upper lid skin crease incision should be made, followed by the brow lift. The remaining degree of excess upper lid skin should then be examined, and a tailored skin excision can subsequently be performed. Worsening of brow ptosis after upper lid blepharoplasty is a difficult problem to fix and may require skin grafting.
