Many different surgical blepharoplasty techniques have been described in the literature. However, there is overall weak evidence-based support for most described approaches.47 A basic limitation of oculofacial research is the variability in skill level of the surgeon for a range of different techniques. What matters most for patient and surgeon is that the postoperative functional and cosmetic outcomes match preoperative expectations. Good cosmetic outcome is highly dependent on the comfort level of the surgeon with a given surgical approach. Thus, what the surgeon knows and is most comfortable with might be more important that the actual choice of technique and surgical approach.
