Sclerotherapy should not be performed in patients who have signs of acute thrombosis or phlebitis, due to the increased risk of deep venous thrombosis. Pregnant patients should defer treatment until after delivery. Diabetes and peripheral arterial occlusive disease (ankleโbrachial index <0.9) are relative contraindications, as they pose a risk for wound healing complications, but this depends upon the nature and extent of sclerotherapy being considered. A history of migraine headache and patent foramen ovale are also relative contraindications to sclerotherapy, due to the risk for microembolism.35โ39 Those with a history of hypersensitivity and allergic reactions to
any of the sclerosants should be pretested to mitigate the risk of anaphylaxis. Sclerotherapy should not be performed to individuals with active infections due to the risk of bacterial spread. Immunocompromised patients and cancer patients receiving chemotherapy should not be treated due to an increased risk of thromboembolic events. Women treated with tamoxifen are at a higher risk for thromboembolic complications during the first 2 years after exposure.40
