For limited excisions the choice between local, sedation (PSA), or general anesthesia is based on the size of the affected area as well as patient and surgeon preferences and the capabilities of the medical center. Due to the maximum dosage of lidocaine allowed, local anesthesia is feasible for lesions up to approximately 1% of BSA.29 Local
anesthesia can be performed as previously described using lidocaine with epinephrine.19 Infiltration should span an area large enough to incorporate the HS lesions with a 1-cm margin. However, anesthetizing an inflamed area is often difficult, and the procedure frequently remains painful under local anesthesia.
