Subcision is another treatment option for rolling scars.74 This procedure can be performed with a filter needle that penetrates the subdermal plane, destroying the scar through forward and backward motion and horizontal rotation of the needle. The motion loosens the fibrotic adhesions that give a bound-down appearance to rolling scars, creating a wound susceptible to collagen deposition. Moreover, loosening the fibrous attachments makes the scars more responsive to other treatments, and promotes neocollagenesis.70
