Ber-EP4 is a monoclonal antibody against human epithelial antigen and binds a glycopolypeptide on the surface and within the cytoplasm of epithelial cells.14 Ber-EP4 is useful in differentiating BCC from SCC, and although the antigen is found in adnexal structures such as apocrine and eccrine glands, aside from the base of the hair bulb, Ber-EP4 is generally absent in hair follicles, making it useful in differentiating BCC from hair follicles.27 Ber-EP4 can be helpful in identifying BCC tumor cells in difficult cases where there is dense inflammation, perineural disease, or an infiltrative component.28
