Lidocaine toxicity must be avoided during MMS. Careful record keeping is critical, especially in large tumors that require several stages to clear. Clinical signs of lidocaine toxicity include talkativeness, perioral paresthesias, tinnitus, vomiting, tremors, seizures, and ultimately cardiopulmonary arrest. To prevent this complication,
the maximum lidocaine dose in adults of 5 mg/kg for plain lidocaine and 7 mg/kg for lidocaine with epinephrine should be observed.
