Patients taking purely elective prophylactic anticoagulants and herbal supplements can be counseled to stop 1 to 2 weeks prior to surgery. Patients should not drink alcohol 24 hours prior to the procedure or 48 hours after the procedure to reduce the risk of bleeding. In patients taking medically necessary anticoagulants, however, the risk associated with postoperative bleeding is significantly lower than the risk of thromboembolic events that may occur if this anticoagulation is ceased.18 Therefore,

most dermatologic surgeons allow patients to continue anticoagulants and antiplatelet medications in the perioperative period. In patients taking coumadin, an INR may be reviewed to ensure it is within the therapeutic window (and preferably three or less).
