The flap is sutured into place with cutaneous sutures starting at the anterior surface of the ear and extending to the helical rim. Basting sutures can be used to recreate the natural contour of the helical rim. This can be accomplished during the first or second stage, but may be easier during the first stage since wound contraction can cause flattening of the pedicle.18 If a full-thickness defect is present, the flap can be folded onto itself to recreate the helical rim. A cartilage graft is rarely needed in this instance unless a large portion of the cartilage is missing. This flap is broad-based and placed without torsion, so it can tolerate some tension without compromising vascularity; minimal to no tension should exist if properly sized.
