Flap design
The location for the flap is selected on the postauricular skin. A template of the defect is
made and transferred to the donor site. The ear should be folded back to the mastoid process and the vertical height of the defect measured to ensure the flap is not undersized. Depending on the size of the wound, the pedicle can involve only the mastoid process or may extend to the postauricular sulcus and posterior ear. A preauricular donor site or other reconstructive options may be considered when patients have a hairline abutting the postauricular sulcus, to prevent the transfer of terminal hairs onto the ear.14
