Wounds that involve the helical rim can be challenging to repair without altering the shape of the ear. When perichondrium is intact, a full-thickness skin graft may effectively restore cosmesis. This is not suitable, however, if only cartilage remains in the wound bed. Though cartilage may eventually heal by secondary intention, there is an increased risk of infection, inflammation, and destructive chondritis. Random patterned, pedicle flaps from the posterior ear provide a reliable alternative for large defects involving the helix and anterior ear.12,14,18 This repair produces a highly aesthetic result that maintains ear size and contour. Additionally, the donor site is hidden from view and heals very well.
