Flap Elevation
An incision is made just lateral to the ala and continued superiorly into the melolabial fold and nasofacial sulcus. This represents the medial portion of the flap and another incision is made laterally to parallel this. The flap is elevated just below the dermis. Transitioning to a deeper plane occurs as the base of the pedicle is approached to preserve the vascular plexus.
