The edges of the defect are squared off to receive the flap, and the flap is thinned to

match the defect. Flap thinning and contouring can be delayed if adequate vascularity is a concern, such as in a smoker. Cutaneous or dermal sutures are used to secure the leading edge of the flap to the defect.
