Shark island pedicle

The shark island pedicle is a useful modification for combined defects that span the ala and perialar cheek. Its advantage is that it provides a single-stage option that preserves the complex contour of the concave cheekโ€“nose junction. While designed in similar fashion to the traditional island pedicle flap, its descriptive name is due to its leading edge having a similar appearance to the mouth and snout of a shark. The superior arm of the flap rotates 90 degrees to repair the alar aspect of the defect while the remainder of the advancing pedicle repairs the cheek up to the nasofacial sulcus.
