Infrequently, unexpected rigidity of the nasal bridge skin may be encountered, even when the patient denies previous surgery in the area. Closure of a wide tertiary defect can occasionally cause distortion of the medial canthus. This can usually be avoided by

accurately assessing the degree of skin laxity prior to design, drawing the secondary lobe slightly narrower than the primary lobe, and making sure all sutures in this area are oriented precisely horizontally.
