A cheek-to-nose flap may bridge the concavity of the nasofacial sulcus as the wound contracts, unless the flap length is adjusted accordingly and tacking sutures are used to recreate the concavity. While this may be corrected with a secondary procedure, it is far better to appropriately design the flap de novo and utilize an appropriate-sized flap with tacking sutures, saving the patient the significant morbidity and cost associated with a secondary procedure.
On the lip, advancement flaps that require excision of the superior standing cone along the base of the nose may ablate the apical triangle and divert the nasolabial fold toward the lateral ala.16,17 If there is no alternative to preserve the apical triangle, patients should be counseled preoperatively that the nasolabial fold may appear to โdive intoโ the nasal sill as opposed to continuing seamlessly onto the nasal sidewall. The natural course of the nasolabial fold can be restored with a Z-plasty.
