Repairs on the scalp should take into account the inelastic nature of scalp skin. Linear repairs are often limited by this inelasticity, though with subgaleal undermining (and possibly galeal relaxing incisions), even fairly large defects may be closed in a linear fashion. There is considerable individual variation in scalp elasticity, and this should always be judged by attempting lateral compression on the scalp in multiple directions prior to closure. Though debatable, considerable tissue creep may be seen in some patients, suggesting a role for intraoperative tissue expansion using either temporary sutures or a towel clamp.
