Persistent bleeding
While electrosurgery is well suited to the cauterization of small blood vessels, larger vessels, and even some smaller arterial vessels, may not respond definitively to electrosurgical approaches. This may be immediately obvious, as when persistent bleeding remains even after repeated attempts at electrocoagulation; more insidiously, some larger vessels will initially appear to be effectively treated with electrosurgical approaches only to persist in bleeding in the postoperative period. Therefore, lessexperienced dermatologic surgeons should err on the side of ligating larger vessels. This may be accomplished in a number of ways; see Chapter 13 for a detailed discussion of suturing techniques for vessel ligation.
