Field (ring) blocks use local anesthesia that is injected circumferentially around the operative site.6 Multiple injections are required with each being placed in an area that was previously anesthetized. Ring blocks are useful in those instances where direct infiltration into the surgical site should be avoided, where nerve blocks are not an option, and for larger surgical areas. In dermatologic surgery, this approach is ideal for the excision of cysts,5,6 skin cancers,14 and when performing hair transplant procedures.14 It is important to inject the local anesthetic in both deep and superficial planes to optimize the numbing effect.5 Ring blocks conserve the total amount of anesthetic that needs to be injected. It is important to wait 5 to 10 minutes after the completion of this block to allow for adequate anesthesia and optimal hemostasis. To maximize hemostasis, the central area may be painlessly infused after completing the ring block.
